    I was in their facility from October through February. My case manager is ********* ********. 

    Before I was discharged, I asked to have discharge papers sent to ****** and *******. I work for ******. ********* and office staff at White Light insisted they forwarded all of the paperwork necessary. 

    Neither ****** nor ******* have received the paperwork they needed. ******* basically wants all of my records. ******'s needs a formal discharge paper with a return to work date.

    Good evening,

    Thank you for reaching out and providing us with the opportunity to resolve this matter.  Upon researching this complaint we discovered a technical problem causing the email attachment to have failed to send.  I have attached the response here which we confirmed was sent and received on 5/22. Previous attempts to send included attempts made on 5/13 and again on 5/14 as well. I have attached those original attempts as well.

    It is always our goal to provide patients with all the details required and we were not aware of the attachment failure.  Thank you for the work you do.  If I can be of any further assistance, please do not hesitate to reach out.
